Adopt a Wounded Warrior Family Holiday Program Application

The Adopt a Wounded Warrior/WII/Gold Star Family Christmas Program was launched in 2013 to help wounded warrior families in our community with temporary assistance or help during the holidays. In 2016 we expanded it to include post 9/11 Gold Star Families. For us to make a determination if your family may be eligible for assistance from our program, we ask that you please take a moment and complete this application form. Please answer all of the following questions for us to better serve you. Those eligible for adoption include U.S. Based active duty military personnel, career or medically retired military, or those honorably discharged from service who have been injured, wounded or acquired a disease during service with a 50% or higher service connection. Priority to post 9/11 service members & gold star families. Post 9/11 Gold Star Families are also welcome to apply** By providing and submitting this form, you are stating that all of the information is true and accurate. Services will not be provided if the organization deems the information false or misrepresented. Applications open 10/1 We HIGHLY advise completing this form on a desktop/laptop computer vs mobile device. If you get an error submitting, please do not attempt to attach files and instead email files to adoptafamily57@gmail.com. The application itself must be received electronically.

  • DD214/DDI 300 & Death Certificate

  • VA Rating Documentation

  • Hardship or other Documentation

Before an application can be considered and action can be taken by the Honor and Courage Foundation, we will need this application and a copy of the following documents mailed or emailed back to our office at Honor and Courage Foundation 960 Gladwyn Rd. Warminster PA 18974 or info@honorandcourage.org.

If Service Member is retired:

  1. A copy of the Service Member’s DD214 showing honorable discharge.

  2. A copy of the Service Member’s VA Letter of Determination

If the Service Member is Active Duty:

  1. A copy of the service member’s most current LES
